How to Apply:
The candidates who are willing to attend the walk-in-interview, for the post of Pharmacist are required to be present at CDM & PHO, Office, Jharsuguda on the aforesaid dates along with their bio-data in special format application form) and attested photocopies of required testimonials along with original documents for verification and beyond the registration time no candidates will be allowed for the same. The appointment will be made purely on merit basis. The candidates who are willing to apply for the post of staff Nurse are requested to apply to office of the CDM & PHO, District Headquarter Hospital, Jharsuguda, pin-768203 on or before 20th September 2018 up to 5.00 P.M through registered post speed post / courier only and the application should be superscribed with the name of the post applied for.
